To me, this is Billy's interpretation of death. It leads to an epiphany of an afterlife that he thought existed before his death (heaven). And kinda like how some people who have near death experiences testify, he was given a choice to either come back to this world or stay in heaven. "And the world so hard to understand/Is the world you can't live without" is his choice of coming back. The last line expresses regret over this decision... a great fucking way to end a song.
